ENTER YOUR NEW EMAIL ID (i.e. your dmClub Fax Number)		@MyFaxNumber.co.uk e.g. 07005699123 or 07005.699123
CREATE A PASSWORD		This is only to administer your Free email address in the future, it does not affect your general dmClub password.
MOTHERS MAIDEN NAME		This is to verify that you are the rightful owner of the dmClub fax number you have entered.
